This list is interesting to me, mostly because it makes me think about how I view these characters. To me, Mac is probably one of the more toxic players/GMs except for Manny at his worst. I can't say for sure why she's worse than, say, DM, who has been shown to be run some nightmare/meat grinder game sessions, other than that Mac just seems to be kind of a jerk at times without being aware of it and doesn't try to change that.
A fair opinion, but from what I have seen, Mac, while a killer DM, doesn't feel as toxic or dangerous to me as those listed above. After all, the players 'rolled in harmony' before Manny took over, so she can't have been THAT bad.
You're probably right, it's just that right now Mac's the one that irks me the most as a player. There are a few reasons for this. One is some of the more recent comics show her focusing almost entirely on the numbers side while seemingly ignoring the roleplaying side. This on its own wouldn't bother me much (different players have different play styles, and s on) but with Mac it feels a bit, mean-spirited I guess (I'm not good with words and expressing myself). There have even been times where it seems like she ignores any problems she causes with other players through her actions, like with Manny during the arc with Blood-bending. This could just my perception (which I'm realizing might be a bit bias as I write this), or it could reflect the group more than the player.

DM definitely earns her rank among the players. Someone who goes out of their way to have their character backstab the rest of the group and then says, "Hey, there were plenty of signs I was gonna betray you, you just didn't do anything about it" has to be the definition of a toxic player.
